Tier 1:
Includes 2 Care Manager visits per month, along with basic care coordination.

Tier 2:
Includes up to 3 visits per month with moderate level of involvement in planning, coordination, and oversight.

Tier 3:
Includes up to 4 Care Manager visits per month, along with specialized oversight, intensive case coordination, and hightouch support.
